Attention Parents of a 6th Grader

Please be advised that the State of New Jersey has issued new requirements regarding immunizations for all students entering 6th grade in September of 2008.  These requirements are as follows:

    1. One dose of Tdap(Tetanus, Diphteria and acellular Pertussis).

    2. One dose of Meningococcal containing vaccine.

Your child will not be allowed to begin school in September of 2008 without meeting these requirements.  If you have any questions, please contact Mrs. Hager at 783-2994 ext. 814.

Attention Parents of Pre-School Students

All students are now required by the State of New Jersey to have the following new immunization:

    1. One dose of PVC(pneumococcal conjugate vaccine) by the start of school in September of 2008.

    2. One dose of the flue vaccine given between September 1st and December 31st.  This will be in effect beginning in September of 2008.

Immunizations

It is of the utmost importance that all immunizations are complete.  The following are required by law before a child can be admitted to school:

Number of shots and type:

    4 - DPT, with booster after 4th birthday.

    3- Polio, with one dose on or after 4th birthday.

    2 - MMR(Measles, Mumps, Rubella on or after 1st birthday.

New Immunization Requirements:

    Every child born on or after January 11, 1990 and entering grade 6 after September 1, 2001 shall have received 3 doses of hepatitis B vaccine. 

Please be advised on August 6, 2002 Governor McGreevey signed a law requiring receipt of hepatitis B vaccine to all pupils in grades 9, 10, 11, and 12 beginning September 2003.

Vaccination Requirements

Pre-K - Varicella or proof of disease.

Kindergarten - Varciella or proof of the disease.

     Any child born on or after January 1, 1998 must have Varciella or proof of Chicken Pox.

 

Required Physical Examinations

    1. One physical is required upon entry to Magnolia School. 

    2. Every student entering Pre K or Kindergarten whichever comes first.

    3. Students in grades 2, 5, 8.

    4. Students playing a sport.

Pre School

Must be 4 years old as of October 1st.

Vaccination requirements:

    4 - DPT  none after 4th birthday.

    3- Polio  none after 4th birthday.

    1 MMR on or after 1st birthday.

    3 - Hepatitus B.

    1 - HIB(Haemophilus Influenzae B).

    1 - Varicella(Chicken Pox)

Acceptable proof of immunizations can be provided in several ways: a signed certificate by a licensed physician, a record from any public health department, or an official school record.

Pupil Medication

    Pupils requiring medication at school must give to the school a written statement from the family physician which identifies the type, dosage, and purpose of the medication.  This includes all medication - prescription and over-the-counter.  There must also be on file in the school, written permission from the parent, to give the prescribed medication to the student.  Medication must be presented in the original labeled container.  This applies for long and short term medication.  Only registered nurses shall administer medication.

Self Medication

    If you wish to have your child self-administer his/her own medication on field trips etc, please follow the instructions below.

    Legislation has been enacted allowing a student to administer his/her own medication.  For a student to do this, certain steps must be taken as per policy #5141.21.  Medication Administration in School.  The policy reads, in part, "Students with asthma or other potentially life-threatening illnesses deemed sufficiently responsible by their physician and parent shall be permitted to have in their possession prescribed medication for the treatment and prevention of life-threatening illnesses during school hours, athletic events, practices and field trips".  The following guidelines and steps need to be followed each year in order to institute this process.

Sports

    Our sports program can be an integral part of your child's school experience.  Magnolia School offers both intramural and interscholastic sports.  To be eligible you must have:

    1. Parental permission.

    2. A physical examination done by your primary Physician or the school physician.

    Only one physical is required per year.  This will enable your child to play any sport for that school year.  Please have the physical done in the beginning of the school year.

       3. A medical questionaire  that updates your child's health status.  One questionaire is required for every sport
